Root Cause Analysis (RCA)

A method used to identify the underlying causes of problems or incidents to prevent their recurrence. RCA is commonly used in QUALITY ASSURANCE and continuous improvement processes. 

Steps in RCA: 

Identify the Problem: Clearly define the issue or incident. 

Analyze the Causes: Use techniques like the 5 Whys or Fishbone Diagram to uncover root causes. 

Implement Solutions: Develop and implement corrective actions to address the root causes.
